Yes, there should be both an Optionable Items WatchList and an Optionable condition available.

In V4 I clicked on Add Rule and when you type Option I dont get Optionable Item

The WatchList is called Optionable Items, the condition is just called Optionable. In any case it appears to be SF5 specific.
What you could do in SF4 is add a column to the WatchList. Choose Flag and then use Optionable Items as the WatchList for the Flag column.
